The HIV rapid Counselor and Tester will provide community engagement and rapid testing throughout New London County on a weekly basis. The C+T will collaborate with Alliance for Living and other community agencies in an effort to recruit as many individuals as possible for rapid testing and eventual linkage to care. The C&T will be responsible for working with the CHC Outreach Coordinator and PrEP Navigator to coordinate outreach and develop and implement an outreach strategy which includes New London County and enhanced rapid HIV C&T. The C&T will work closely with the CKP team to establish a process for the linkage and retention in care of any individual who tests positive for HIV or other STI’s.



Become an expert in rapid HIV testing including all aspects of counseling for individuals at risk of HIV transmission.

Provide HIV/HCV rapid testing to the New London community with the goal of performing at least 25-30 rapid HIV tests per week.

Provide education, awareness and support to the New London community around HIV risk reduction, transmission risk and community viral load.

Attend community events that serve the target population for HIV testing and provide rapid HIV testing, as well as, education.

Work with CHC communications team to design and implement an HIV testing campaign designed to target those individuals at highest risk for HIV.

Establish a seamless process for linkage to care for those individuals who test positive for HIV and work with providers to ensure appropriate referral and follow up within 7 days of diagnosis.

Establish a seamless process for linkage to care for individuals in the community who request primary care services.

Track referrals to HIV and primary care on a regular basis.

Use appropriate data system to enter all clinical information as necessary for patient care.

Collaborate closely with the CHC CKP team to organize events, promote involvement in services and track follow up.

Participate in CKP QM projects and sessions as requested by CKP leadership.
