The Community Health Worker (CHW) is an integral member of the interdisciplinary health team at Community Health Center, Inc. (CHCI). The CHW will engage community members to offer an array of resources related to the broad scope of health care and data tracking with optimal breast/cervical/cardiovascular health education and care coordination for potential female patients identified in the field. Our ultimate goal is for our program participants to establish primary care in one of our identified CHCI patient centered medical homes. The CHW will navigate care for community participants at the point of entry into CHCI’s care delivery system while assisting our clinical teams, where appropriate in the Meriden and Middletown CT sites. The CHW will understand the clinical pathways established by their local community hospitals and will direct the potential patients to health care services within the organization at CHCI and at outside facilities if necessary for timely diagnosis and treatment. The CHW is responsible for completing basic health assessments, patient referral and follow-up, and facilitating verbal and written communication for staff and patients when necessary. This position is responsible for navigating and promoting access to the Connecticut Early Detection and Prevention Program (CEDPP), providing extensive education regarding breast, cervical and cardiovascular health, and striving for a preventive approach with all potential patients. The CHW will connect patients with resources, health care and support services in their communities and provide continuous health coaching while enrolled and active under the CEDPP program. This position works closely with the Navigation team and reports directly to the Program Manager of assigned project.

The Community Health Worker is responsible for:

  • Completing basic health assessments
  • Patient referrals and follow-ups
  • Facilitating verbal and written communication for staff and patients when necessary
  • Navigating and promoting access to the Connecticut Early Detection and Prevention Program (CEDPP)
  • Providing extensive education regarding breast, cervical and cardiovascular health, striving for a preventive approach with all potential patients
  • Connects patients with resources, health care and support services in their communities and provide continuous health coaching while enrolled and active under the CEDPP program.
  • Works closely with the Navigation team and reports directly to the Program Manager of assigned project. 
  • Due to the communities and patient population this role will support, bilingual (English/Spanish) is required for this role
  • NOTE:  For CHC internal applicants, commensurate work experience may be substituted for the degree
