Clinical Responsibilities:

  • Administers alcohol and drug screens to CHC patients using electronic screening tools in EHR.
  • Provides patients with brief intervention (counseling) as appropriate using evidence-based treatment approaches.
  • Completes referrals internally for patients with screening results indicating need for referral to drug/alcohol treatment by coordinating with RN, BH Provider or Medical Provider.
  • Promotes maintenance of healthy behaviors for all patients with screening results indicating no current need for brief intervention or referral to treatment.
  • May participate in integrated care meetings to coordinate care and refer patients to complex care management when appropriate.
  • Participates in trainings related to SBIRT project as required, participates in clinical and administrative QI activities involving SBIRT project, provides feedback regarding SBIRT work-flow and ensures efficient and effective implementation.


Administrative Responsibilities:

  • Accurately and completely documents the clinical encounter per CHC training and instruction. 




Associate of Science (A.S.): Nursing Preferred
