We use the latest treatments and technologies within the Patient-Centered Medical Home Model (PCMH) to care for our growing patient base.As a Access to Care Coordinator for Community Health Center, you will:This is the opportunity for you if you:Are committed to providing superior care and service to children, adolescents and familiesWould like to make a huge impact in someone's lifeAre a compassionate individual who wishes to improve healthcare access for the underservedWant to work under excellent supervisionEnjoy being a part of an outstanding team of professionalsAs an Access to Care Coordinator, you will:Act as the initial point of contact for all new CHC patients and other clients referred from the community Conduct needs assessments to identify barriers to obtaining healthcareMain coordinator of direct services with clients and patients (no supervisory responsibilities)Assist clients in establishing a primary medical and dental homeRegister all new medical patients in database and provide an orientation to the practiceProvide application assistance for private and public assistance programs, such as HUSKYAssure each application is reviewed and final determination is completed on all applicationsAdvocate and make referrals for additional, needed health and social servicesConduct community outreach and provide staff education on healthcare access issuesTrack all Access to Care Activity in a specialized databaseCollaborate with others within the Community Health Center team in order to provide total and complete careCommunity Health Center, Inc. is an Equal Opportunity Employer.
